Our initial callback form asks for your full name, mobile number, email address and the allied health service you would like to access. You may also provide a brief optional enquiry. Please do not submit health information, DVA details or other sensitive information through this form.

We use the information you provide to respond to your enquiry, discuss the service you are seeking and arrange appropriate next steps. Where referral assistance is requested, we will explain any proposed involvement of an independent referral provider and obtain appropriate consent before sharing additional information.
Disclosure and service providers
We do not sell your personal information. Website form submissions may be transmitted and temporarily processed by our form-delivery and email service providers so the enquiry can be delivered to us. Information may otherwise be disclosed to service providers who support our operations, where necessary and subject to appropriate privacy and security arrangements, or where disclosure is authorised or required by law.
